Cervical collar or cervical pillow? Soft collar or Philadelphia collar?

By Humable Ortho Insights team · Last updated · 2 min read

Neck supports are easy to confuse. They range from soft foam collars to firm two-piece collars and supportive pillows. Here is what each is for.

Cervical collars: for limiting movement

A collar wraps around the neck and limits how far it can move. Doctors suggest them for short periods after a neck injury such as whiplash, after neck surgery, or during a painful flare-up. A collar is not a first choice for ordinary neck stiffness.

Soft foam collar

The Humable Cervical Collar uses medium-density foam with a stockinette cover for hygiene, and gives firm but comfortable support. It suits gentler needs, such as after whiplash or a painful neck, when a doctor advises support.

Collar with chin support

The Cervical Collar Support is designed around chin support, to hold the neck steady or in a slightly raised position. It is used in situations such as after neck surgery or with cervical spondylitis and similar conditions, when advised.

Philadelphia collar

A Philadelphia-style collar has moulded foam and a firmer structure. It limits forward and backward movement more than a soft collar, and supports the chin. It is used after procedures such as discectomy or spinal fusion, or when immobilisation is advised.

Soft collar Collar with chin support Philadelphia collar
Restriction Gentle Moderate Firmer
Material Foam, cotton Foam, cotton, nylon Moulded foam, plastic
Typical use Painful neck, whiplash Post-surgery support, spondylitis Post-surgery, immobilisation

Cervical pillow: for comfort

A cervical pillow supports the neck and keeps it comfortably positioned. It is for comfort and for people who want gentle support. It does not restrict movement the way a collar does.

Why not to wear a collar for too long

Wearing a collar longer than advised can leave neck muscles weaker and stiffer. Wear it for the time your doctor recommends and ask what exercises to do.

See a doctor immediately if

  • Neck pain follows an accident or fall
  • You have numbness, tingling or weakness in your arms
  • You feel dizzy, or have trouble swallowing

A collar cannot tell you whether an injury is serious.

Frequently asked questions

What is the difference between a cervical collar and a cervical pillow?

A collar wraps around the neck to limit movement and is used on medical advice. A pillow gives comfort and support but does not restrict movement.

What is a Philadelphia collar used for?

It is a firmer collar with moulded foam that limits forward and backward movement and supports the chin. It is used after procedures such as discectomy or spinal fusion, or for immobilisation, on medical advice.

How long can I wear a cervical collar?

Only as long as your doctor or physiotherapist advises. Wearing one for too long can leave the neck weaker.

Can I sleep in a cervical collar?

Only if your doctor has said so. Some conditions need night-time support and others do not.

Should I use a collar for a stiff neck?

For ordinary stiffness a collar is usually not needed. See a doctor if the neck pain is severe, follows an accident or does not settle.
